Request Your Medical Records Online
- Patients: If you are our patient and would like to request your medical records, please click on the "Begin" link below to complete your request. You will be asked to provide a valid email address and a copy of your ID as part of the process.
- Third Party Request: This option is for healthcare providers, attorneys, etc. Click on the "Begin" link below, fill out the request details, and upload your requests/authorizations.Please do not upload subpoenas or court order, all subpoenas and court orders must be submittedto the facility directly.
- Note: You may be contacted to provide additional documentation, such as a birth certificate, power of attorney, etc. You will receive an update or response from Sharecare within 3 business days.
